| Field | Description |
| Category | If you have created categories, you can select a category for this survey to belong to |
| Type | This is an optional field inherited from an earlier version of PegaSurvey. You can enter a text value here for reporting purposes, but its use is discouraged. The field does not appear in the My Surveys landing page tab. |
| Hide Label | Check the check box to hide the label. |
| Hide Instructions | Check the check box to hide the label. |
