Summary
A transaction error occurs sporadically when attaching a document to a work object.

Steps to Reproduce
Attach a document.
Root Cause
An issue in the custom application code or rules.
Multiple Refresh Section calls are triggered from the same control. One of these Refresh Section calls, invokes an activity that triggers a commit.
Resolution
Perform either of the following local-changes:
- Utilize an activity that does not perform a commit. When an action other than the work object or assignment absolutely must be persisted to the database without waiting for flow processing, use an Obj-Save with the Write-now option.
- Perform an additional refresh section through other means, outside the scope of the control that triggers the refresh who's activity calls the commit.
