Audit Trial must be updated only once for every success stage

SA-13351

Summary

Audit Trail is updated according to the number of submit actions taken, not based on the number of successful submit actions taken.
For example, if a flow action is submitted without answering the mandatory questions, and then it is submitted again with all the mandatory questions, there are two entries in the audit log.
