Extra remote case is found when new application is created

Summary
An extra remote case is added with a blank "Mid Term Amendment" type when App Express is run on top of Customer Service for Insurance.

Steps to Reproduce
- Run the Application wizard and create a new application on top of the CSI 07.21 application.
- Once the application is created, notice the extra remote case in the case overview section.
Root Cause
A defect in the pyExtendedLoadCaseHierarchy activity. The activity has been updated to show all the available remote cases.
Resolution
Apply HFix-30287 and the local change, which updates the activity pyExtendedLoadCaseHierarchy.
The local change includes two new steps which perform a Page-Remove if specific conditions are met as exhibited below:
