Hfix 9989 isn't working as expected for RC4808 CC7

Summary
As provided in the release notes for HFix-9989, questions are to be displayed in the same manner for both RC4808 CC6 and CC7. For CC6 the questionnaire is as expected but for CC7 they are not. Questionnaire should have been same for both CC6 and CC7 reason codes.

Steps to Reproduce
1) Create a dispute case for master card.
2) In choose reason code screen select reason code as 4808 and condition code as 7.
3) Go to ancillary questions screen to observe problem.
Root Cause
The root cause of this problem is an error/omission in product documentation. There was incorrect information in the documentation.
Resolution
This is just a typo. Both changes mentioned in the readme for HFix-9989 are for 4808 CC6, CC7 is not affected.
