Operator avatars uploaded are not successfully saved

Summary
Sporadically, operator avatars uploaded are not successfully saved. The operator avatar is changed on the RuleForm, however, after it is saved, the avatar is reverted to the default silhouette.
Error Messages
Not Applicable.
Steps to Reproduce
- Open an operator instance
- Click 'Upload image'
- Click 'Ok' on the address bar and click the Save button of the form
- Repeat the above step for a few times and click 'Upload image'
- Click 'Ok' on the address bar and click the Save button of form.
Root Cause
A defect in Pegasystems’ code or rules.
Resolution
Apply HFix-45296.
