Steps to Reproduce

1. Open a Case.
2. Complete the Checklist.
3. Enter the Comments.
4. Attach a File.
5. Submit the Checking Case.

Root Cause

For one of the instances, a custom activity was calling commitWithErrorHandling where not required.

Resolution

Change in custom activity to remove call to commitWithErrorHandling where not required.
