Unable to access the Operator ID page on logging in through LDAP.

Steps to Reproduce
Log in through LDAP and create a new workobject.
Root Cause
In the user authentication activity, the user has created a page named Operator ID which is also generated by the Pega application. As a result, the Pega Operator ID page was overridden.
Resolution
Perform the following local-change;Change the name of the Operator ID page in the custom activity.
