Integration with MCG Informed by MCG for DM

Seamless integration between Pega Care Management and MCG’s Informed by Cite Care for DM provides clients with access to industry-leading, evidence-based assessments, care plan elements and educational content. With this integration, health condition focused assessments, care plan elements and educational content are presented within the workflow. The application integration dynamically generates care plan recommendations and provides the user the ability to personalize the content and care plan with the patient. All elements are stored in the patient record as a basis for comparison and evaluation with real-time, bidirectional interactions with the MCG product.
